義母（子供の祖母）が、今年のクリスマスとニューイヤーに子供をアメリカに一緒に連れて行ってくれるのですが、そのときに必要なものはなんですか？

両親（私と主人）の手紙は必要ですよね？
あと、祖母と子供の苗字が違うのですが、なにか問題になりますか？

両親の同意書は必要です。その同意書の中に、自分の子供の名前とパスポート番号、自分の子供に同行する人の名前とパスポート番号（身分証明書の番号）を書いておけばいいのです。おばあちゃんだろうか、おばさんだろうか、お姉さんだろうが、両親が一緒に同行していないかぎりは必要な書類です。なので、苗字がおばあさんと子供がおなじかおなじでないかは関係なく、同意書にかかれている、子供と同行する大人の名前とそのときに一緒にいる人が一致していればいいわけです。 　

さらにその同意書をノタリーパブリックへ持参して署名は保証人の前でしないと正式なドキュメントにはなりませんよね？
そうでないと誰でも書けるわけですから。

正式な書類にする為にはＮｏｔａｒｙ　Ｐｕｂｌｉｃ（公証人）前でのサインが必要です。

これは、両親のどちらかが同行せずに子供を連れてカナダ国外に出る際に、同行しない親が、同行する親に子供を連れて行くことを許可する手紙の例です。

この例に沿って手紙を完成させ、弁護士または、Ｎｏｔａｒｙ　Ｐｕｂｌｉｃ（公証人）の前でサインするこによって公証することをお薦めします。同行する親がこの手紙を子供のパスポートとともに必ず携帯してください。
